Ai Kiyoshi (愛藍淳; Kiyoshi Ai; lit. Love peace ) is Captain of 4th Division and Chief Medical Officer in most Hollow Operations. She was moved up from 4th Chair to Captain when Unohana Retsu transferred to Division Zero, along with Sui-Feng and Byakuya. She was offered the position because of her master healing techniques and her closeness to her captain, also her amazing willpower.

Ai is a woman of average height, with long black hair usually put with a white ribbon. She has black colored eyes, claiming that it has always been so with the Kiyoshi Clan. her outfit matches that of a Miko, which is ironic for a spirit, a long sleeved jacket with slits at the shoulders and cords on the sleeves. Also a long red cloth skirt that goes down from her waist to her ankles.
She also wears long white socks and black walking shoes, her zanpakuto is often on the left side of her waist and she manages to draw it using her left hand. Her right hand is used for healing Kido and is covered by a black glove or hidden under her sleeve.
She also wears a string necklace around her neck that, when Hollows are about, raises and points in the direction as she is unable to find them herself. Her body, underneath is covered in multiple cuts, scars, and bruises though this is merely a side-effect of her zanpakuto's abilities.
